These questionnaires help validate and develop PortoCare. Participation is voluntary and is not a booking or a commitment to join the pilot.

Who is responsible

What is collected and why

We store your questionnaire answers, submission date and language. A random session identifier helps prevent duplicate or abusive submissions. We do not ask for patient details or clinical information. Name, organisation and email are optional, and are stored only when you explicitly agree to be contacted about validation and the future pilot.

Choice and access

You can submit your answers without identifying yourself. Providing contact details is a separate choice. The answers are accessible only to the project promoter through a protected area. This site uses OpenAI Sites and Cloudflare for hosting and storage. We do not sell your answers or use them to send unrelated commercial messages.

Retention and your requests

We plan to keep responses for up to 12 months. You can ask for access, correction or deletion of identifiable data, or withdraw your permission to be contacted, by emailing the address above. Anonymous responses may not be identifiable after submission; keep the submission reference if you want to request their deletion.

Session cookie

The questionnaire uses a temporary cookie, lasting one hour, to protect form submissions. It is not used for advertising or analytics.
